Every instrument sent for repair must include this form. Incomplete information can delay the service and return of your equipment. 

Payment for all services must be made by credit card or check before the instrument is returned, unless otherwise arranged.

Please Note: If noticeably contaminated, monitors must be cleaned, sanitized, or otherwise decontaminated to protect our employees from health risks from handling potential hazardous material. Thank you!

IMPORTANT INFORMATION

• Clicking the Submit Form button is required for your service request to be received.

• There is a $50 diagnostic charge per instrument. This fee is waived only if estimate is accepted and instrument is repaired.

• There is a decline/return or surrender fee of $95 per each estimate that is declined in its entirety.

• Shipping Charges are additional and are added to the invoice.

• Payment must be by credit card, ACH, or check only
